Please fill out the following form for your scholarship application. To be considered for the Delbert Bavier’ Madison II Scholarship, students must meet the following criteria:
High school seniors with a GPA of 3.0 or higher who have persevered through significant life challenges are eligible to apply. Applicants must be planning to attend college and be able to provide proof of admission and enrollment to a public or private college/university.
Your complete application must include an official transcript showing a GPA of 3.0 or higher, two recommendation letters from academic teachers, counselors, or principals at your current high school, proof of admission and enrollment to your college/university, and an essay of at least 250 words describing your goals, challenges you've faced, and why you deserve this scholarship.

The selection committee reviews applications from students who have persevered through life challenges, much like Delbert Madison II did throughout his life with Duchenne Muscular Dystrophy. We look for students who demonstrate determination, resilience, and a positive attitude despite obstacles. Academic achievement, community involvement, and the quality of your personal essay are all considered in making our selections.
