Please fill out the following form for your scholarship application. To be considered for the Delbert Bavier’ Madison II Scholarship, students must meet the following criteria:

GPA of 3.0 or higher with transcript attached
Two recommendation letters from an academic teacher, counselor, or principal at current high school
Proof of admission and enrollment as a degree seeking student to a public or private college/ university (Funds made payable to applicable college/university.)
Essay of 250 words or more in which you describe your goals/ aspirations, discuss the challenges or adversities faced in your life, and explain why you should be deserving of this scholarship.
